Personal Care Services can include:

  • Grooming and Dressing Support

  • Routine Hair and Skin Care

  • Escorting to Appointments

  • Arranging Transportation

  • Medication Reminders

  • Mobility Assistance (Transferring and Ambulation)

Eligibility Requirements:

​

  • Approved Medicaid coverage

  • A diagnosis of a long-term physical or mental disability

  • Need for assistance with Activities of Daily Living (ADLs)

  • Preference to receive care while staying comfortably at home
